OUTREACH
INFORMATION and ASSISTANCE
Appointments will be taken by Whiteside County Senior Center representatives for
Information and Assistance with Illinois Cares Rx/Circuit Breaker, Social Security
Extra Help, Medicare Part D, LIHEAP (energy assistance), ITAC (free amplified
phone program) and Property Tax Assessment Freeze at the following locations
throughout Whiteside County during the month of:February:
Feb 2 Riverbend Senior Center and Transit Facility ,Fulton
(old fire station) 10:00-11:00
Feb 13 Odell Public Library, Morrison 10:00-11:00
Feb 14 Prophet Manor, Prophetstown 9:30-10:30
Feb 15 Erie Public Library 9:00-10:00
Feb 16 Riverbend Senior Center and Transit Facility, Fulton
(old fire station) 10:00-11:00
Feb 22 ampico Area Community Bldg, Tampico 10:00-12:00
Feb 27 Odell Public Library, Morrison 10:00-11:00
If you require assistance at the Senior Center or at one of the above named sites,

WCSC Outreach Departments are also available, Monday through Friday, to assist
clients at the Whiteside Senior Center, 1207 W. Ninth St., Sterling (or through home
visits for clients not capable of coming in).
When you come in for your appointment at one of the above named Senior Centers
or sites please bring the following documentation with you for the completion of
Form IL-1363 Illinois Cares Rx/Circuit Breaker:
All 2010 income including Social Security benefits, pensions, interest, wages, etc, a
rent receipt with landlord's name, address, phone, or 2009 real estate tax bill, your
Medicare card, Medicare D card, Medicaid card and value of assets (Bank accounts,
CD's, investments, life insurance cash value, investment property, etc.)
When applying for energy assistance you must have the following items with you
when you come in for your appointment:
Social Security cards for everyone in the household, proof of all household income
for the last 30 days (bank statement, check stubs, etc.) and most recent light and gas
bill.
