Appointments will be taken by Whiteside County Senior Center representatives for Information and Assistance with Illinois Cares Rx/Circuit Breaker, Social Security Extra Help, Medicare Part D, LIHEAP  (energy assistance), ITAC (free amplified telephone program) and Property Tax Assessment Freeze at the following locations throughout Whiteside County every month:

WCSC Outreach Departments are also available, Monday through Friday, to assist clients at the Whiteside Senior Center, 1207 W. Nineth St., Sterling ( or through home visits for clients not capable of coming in ).

 

 

When you come in for your appointment at one of the above named Senior Centers or sites please bring the following documentation with you for the completion of Form IL-1363 Illinois Cares Rx/Circuit Breaker:

 

 

 

All 2010 income including Social Security benefits, pensions, interest, wages, etc, a rent receipt with landlord’s name, address, phone, or 2010 real estate tax bill, your Medicare card, Medicare D card, Medicaid card and value of assets (Bank accounts, CD’s, investments, life insurance cash value, investment property, etc.

 

 

When applying for energy assistance you must have the following items with you when you come in for your appointment:

 

Social Security cards for everyone in the household, proof of all household income for the last 30 days ( bank statement, check stubs, etc. ) and most recent light and gas bill.

 

 

Whiteside County Senior Center is a United Way of Whiteside County funded agency.
