Illinois Cares Rx is a state pharmaceutical assistance program.  It can pay your Medicare Part D premium if you are in a coordinating plan, and lowers your co pays, or if you have other coverage other than Medicare Part D it will give you a twenty five dollar a month check to help with your prescription cost if you qualify.  Circuit Breaker provides cash grant assistance with your property tax/rent and a license plate discount if you qualify. You must be 65 year old or you may be eligible if you are 16 years of age or older before January 1, 2010 and totally disabled.Online applications for Illinois Cares Rx/Circuit Breaker are scheduled to begin on January 13, 2010.  The website for the online application through the Illinois Department on Aging is:  www.cbrx.il.gov . The Lee County Council on Aging and the Whiteside County Senior Center can help anyone who would like assistance with this application process. Both Senior Centers are taking appointments for assistance at different sites around the two counties as well as their respective Senior Centers.  A general listing is on this site and we advertise monthly sites in local newspapers and in the monthly Golden Echo inside the Daily Gazette.

 

 

Please bring the following documentation with you for the completion of Form IL-1363 Illinois Cares Rx/Circuit Breaker application.

 

All 2009 income including Social Security benefits, interest, dividends, wages, etc., a rent receipt with landlord’s name, address, phone, or 2009 real estate tax bill, your Medicare card, Medicare D card, Social Security card and value of assets such as: (Bank accounts, CD’s, investments, life insurance cash value, investment property, etc.
