IL 1363-Illinois Cares RX and Circuit Breaker
Filing deadline is December 31st


Illinois Cares Rx is a state pharmaceutical assistance program. It can pay your Medicare Part D premium if you are in a coordinating plan, and lowers your co pays if you qualify. Circuit Breaker provides cash grant assistance with your property tax/rent and a license plate discount if you qualify. You must be 65 years old or you may be eligible if you are 16 years of age or older before January 1, 2012 and totally disabled. You can apply online for Illinois Cares Rx/Circuit Breaker. The application is available through the Illinois Department on Aging website at www.cbrx.il.gov. This application must be filed every year preferably before October to retain your prescription benefits for the following year. The Whiteside County Senior Center can help anyone who would like assistance with this application process.

Please bring the following documentation with you for the completion of Form IL-1363 Illinois Cares Rx/Circuit Breaker application:
All 2011income including Social Security benefits, interest, wages, etc, the amount you paid for rent in 2011, the landlord's name, address, phone, or 2011 real estate tax bill, your Medicare card, Medicare D card, Social Security card and value of assets such as; (Bank accounts, CD's, any kind of investments, investment property, etc.)
